In this consensus report by a diverse group of academics who conduct and/or are concerned about social and behavioral genomics (SBG) research, the authors recount the often‐ugly history of scientific attempts to understand the genetic contributions to human behaviors and social outcomes. They then describe what the current science—including genomewide association studies and polygenic indexes—can and cannot tell us, as well as its risks and potential benefits. They conclude with a discussion of responsible behavior in the context of SBG research. (...) SBG research that compares individuals within a group according to a “sensitive” phenotype requires extra attention to responsible conduct and to responsible communication about the research and its findings. SBG research (1) on sensitive phenotypes that (2) compares two or more groups defined by (a) race, (b) ethnicity, or (c) genetic ancestry (where genetic ancestry could easily be misunderstood as race or ethnicity) requires a compelling justification to be conducted, funded, or published. All authors agree that this justification at least requires a convincing argument that a study's design could yield scientifically valid results; some authors would additionally require the study to have a socially favorable risk‐benefit profile. Obtaining prospective written consent from women to participate in trials when they are experiencing an obstetric emergency is challenging. Alternative consent pathways, such as gaining verbal consent at enrolment followed, later, by obtaining written consent, have been advocated by some clinicians and bioethicists but have received little empirical attention. We explored women’s and staff views about the consent procedures used during the internal pilot of a trial, where the protocol permitted staff to gain verbal consent at recruitment. Interviews with staff (...) and participating women. Data were analysed thematically and interviews were cross-compared to identify differences and similarities in participants’ views about the consent procedures used. Women and some staff highlighted benefits to obtaining verbal consent at trial enrolment, including expediting recruitment and reducing the burden on those left exhausted by their births. However, most staff with direct responsibility for taking consent expressed extreme reluctance to proceed with enrolment until they had obtained written consent, despite being comfortable using verbal procedures in their clinical practice. To account for this resistance, staff drew a strong distinction between research and clinical care and suggested that a higher level of consent was needed when recruiting into trials. In doing so, staff emphasised the need to engage women in reflexive decision-making and highlighted the role that completing the consent form could play in enabling and evidencing this process. While most staff cited their ethical responsibilities to women, they also voiced concerns that the absence of a signed consent form at recruitment could expose them to greater risk of litigation were an individual to experience a complication during the trial. Inexperience of recruiting into peripartum trials and limited availability of staff trained to take consent also reinforced preferences for obtaining written consent at recruitment. While alternative consent pathways have an important role to play in advancing emergency medicine research, and may be appreciated by potential recruits, they may give rise to unintended ethical and logistical challenges for staff. Staff would benefit from training and support to increase their confidence and willingness to recruit into trials using alternative consent pathways. Elephants show a rich social organization and display a number of unusual traits. In this paper, we analyse reports collected over a thirty-five year period, describing behaviour that has the potential to reveal signs of empathic understanding. These include coalition formation, the offering of protection and comfort to others, retrieving and 'babysitting' calves, aiding individuals that would otherwise have difficulty in moving, and removing foreign objects attached to others. These records demonstrate that an elephant is capable of diagnosing animacy (...) and goal directedness, and is able to understand the physical competence, emotional state and intentions of others, when they differ from its own. We argue that an empathic understanding of others is the simplest explanation of these abilities, and discuss reasons why elephants appear to show empathy more than other non-primate species. Patients who are dying of cancer usually give up eating and then stop drinking. This raises ethical dilemmas about providing nutritional support and fluid replacement. The decision-making process should be based on a knowledge of the risks and benefits of giving or withholding treatments. There is no clear evidence that increased nutritional support or fluid therapy alters comfort, mental status or survival of patients who are dying. Rarely, subcutaneous fluid administration in the dying patient may be justified if the (...) family remain distressed despite due consideration of the lack of medical benefit versus the risks. Some cancer patients who are not imminently dying become dehydrated from reversible conditions such as hypercalcaemia. This may mimic the effects of advanced cancer. These conditions should be sought and fluid replacement therapy should be given along with the specific treatments for the condition. (shrink)

Background and objective: Code status discussions may fail to address patients’ treatment-related goals and their knowledge of cardiopulmonary resuscitation (CPR). This study aimed to investigate patients’ resuscitation preferences, knowledge of CPR and goals of care. Design, setting, patients and measurements: 135 adults were interviewed within 48 h of admission to a general medical service in an academic medical centre, querying code status preferences, knowledge about CPR and its outcome probabilities and goals of care. Medical records were reviewed for clinical information (...) and code status documentation. Results: 41 (30.4%) patients had discussed CPR with their doctor, 116 (85.9%) patients preferred full code status and 11 (8.1%) patients expressed code status preferences different from the code status documented in their medical record. When queried about seven possible goals of care, patients affirmed an average of 4.9 goals; their single most important goals were broadly distributed, ranging from being cured (n = 36; 26.7%) to being comfortable (n = 8; 5.9%). Patients’ mean estimate of survival to discharge after CPR was 60.4%. Most patients believed it was helpful to discuss goals of care (n = 95; 70.4%) and the chances of surviving inhospital CPR (n = 112; 83.0%). Some patients expressed a desire to change their code status after receiving information about survival following inhospital CPR (n = 11; 8.1%) or after discussing goals of care (n = 2; 1.5%). Conclusions: Doctors need to address patients’ knowledge about CPR and take steps to avoid discrepancies between treatment orders and patients’ preferences. Addressing CPR outcome probabilities and goals of care during code status discussions may improve patients’ knowledge and influence their preferences. (shrink)

Interactive narratives are inextricable from the way that we understand our encounters with digital technology. This is based upon the way that these encounters are processually formed into a narrative of episodic events, arranged and re-arranged by various levels of agency. After describing past research conducted at the iCinema Research Centre at the University of New South Wales, this paper sets out a framework within which to build a relational theory of interactive narrative formation, outlining future research in the area.

The capacity to be creative, to produce new concepts, ideas, inventions, objects or art, is perhaps the most important attribute of the human brain. We know very little, however, about the nature of creativity or its neural basis. Some important questions include how should we define creativity? How is it related (or unrelated) to high intelligence? What psychological processes or environmental circumstance cause creative insights to occur? How is it related to conscious and unconscious processes? What is happening at the (...) neural level during moments of creativity? How is it related to health or illness, and especially mental illness? This paper will review introspective accounts from highly creative individuals. These accounts suggest that unconscious processes play an important role in achieving creative insights. Neuroimaging studies of the brain during "REST" (random episodic silent thought, also referred to as the default state) suggest that the association cortices are the primary areas that are active during this state and that the brain is spontaneously reorganising and acting as a self-organising system. Neuroimaging studies also suggest that highly creative individuals have more intense activity in association cortices when performing tasks that challenge them to "make associations." Studies of creative individuals also indicate that they have a higher rate of mental illness than a noncreative comparison group, as well as a higher rate of both creativity and mental illness in their first-degree relatives. This raises interesting questions about the relationship between the nature of the unconscious, the unconscious and the predisposition to both creativity and mental illness. (shrink)

Philosophers, especially in recent years, have engaged in reflection upon the nature of experience. Such reflections have led them to draw a distinction between conscious and unconscious mentality in terms of whether or not it is like something to have a mental state. Reflection upon the history of psychology and upon contemporary cognitive science, however, identifies the distinction between conscious and unconscious mental states to be primarily one which is drawn in epistemic terms. Consciousness is an epistemic notion marking the (...) special kind of first-person knowledge we have of our own mental states. Psychologists have found it expedient, for explanatory reasons, to ignore or reject the assumption that we have exhaustive first-person knowledge of our mental states and, in doing so, use the term 'unconscious' to indicate the peculiar epistemic status of certain mental states.It is argued that epistemic consciousness is distinct from the subjective-experiential notion of consciousness, from 'access-consciousness' and from higher-order thought conceptions of mental state consciousness, and that epistemic consciousness has an important role to play in philosophy of mind and in the history of psychology. The prospect of dealing with a rapidly and inexorably bleeding patient fills most medical practitioners with alarm. When that patient is a Jehovah's Witness, the knowledge that a blood transfusion is likely to be refused turns that alarm into a state of acute anxiety and conflict. This state is further heightened when the patient is young and otherwise healthy--a situation found particularly in obstetric practice with the occurrence of ante- and post-partum haemorrhage, and ectopic pregnancy. In the last 25 years (...) in England, Wales, Scotland and Northern Ireland, there has been one maternal death in which the refusal to accept a blood transfusion has been considered to be an avoidable factor. In this article I have attempted to identify the magnitude of the problem in obstetric practice and have sought to clarify the moral and legal aspects. (shrink)

The traditional verbal/nonverbal dichotomy is inadequate for completely describing cerebral lateralization. Musical functions are not necessarily mediated by the right hemisphere; evidence for a specialist left-hemisphere mechanism dedicated to the encoded speech signal is weakening, and the right hemisphere possesses considerable comprehensional powers. Right-hemisphere processing is often said to be characterized by holistic or gestalt apprehension, and face recognition may be mediated by this hemisphere partly because of these powers, partly because of the right hemisphere's involvement in emotional affect, and (...) possibly through the hypothesized existence of a specialist face processor or processors in the right. The latter hypothesis may, however, suffer the same fate as the one relating to a specialist encodedness processor for speech in the left. Verbal processing is largely the province of the left because of this hemisphere's possession of sequential, analytic, time-dependent mechanisms. Other distinctions are special cases of an analytic/holistic dichotomy. More fundamentally, however, the left hemisphere is characterized by its mediation of discriminations involving duration, temporal order, sequencing, and rhythm, at thesensory level, and especially at themotorlevel. Spatial aspects characterize the right, the mapping of exteroceptive body space, and the positions of fingers, limbs, and perhaps articulators, with respect to actual and target positions. Thus there is a continuum of function between the hemispheres, rather than a rigid dichotomy, the differences being quantitative rather than qualitative, of degree rather than of kind. (shrink)

A short review is given of the Nuffield Council’s report on behavioural genetics. This review is used as an entry point to a discussion of the factors that influence the presentation of behavioural genetics in the media and in the popular scientific press. It is argued that our interest in formulating narrative explanations of our individual lives puts pressure on publishers and editors to present behavioural genetics in a selective, misleading, way. Some other influences on presentation are discussed and it (...) is suggested that the Nuffield report is particularly useful in so far as it lacks these distorting influences. (shrink)

Stroud’s aim is threefold: 1) to provide an introduction for beginners, 2) "something of interest to Hume scholars," and 3) to "philosophers dealing with the problems he discussed". Stroud dissociates himself from those who regard Hume merely as a sceptic, from those who view Hume as a precursor of positivism, and from those who attribute to Hume an interest in analyzing or defining concepts. Instead, Stroud subscribes wholeheartedly to the Kemp Smith theses, and he emphasizes Hume’s positive theory of a (...) naturalistic science of man as a "completely comprehensive empirical investigation and explanation of why human beings are the way they are, and why they think, feel and behave as they do." Stroud then proceeds to interpret selected aspects of Hume with reference to the foregoing aim in conjunction with the "theory of ideas." The result is to see Hume’s positive theory as a form of projection, and some attempt is made to explore the relationship between an empirical investigation and a theory of projection. (shrink)

In pursuit of the first aim, Kekes directs his book to the attention of Anglo-American philosophers who have forsaken the key philosophical task. His approach is twofold. Writing in a post-Kuhnian period, he rebuts those versions of philosophical theorizing predicated on the distinction between discovery and justification. At the same time he presents an incisive running critique of scientism and the reduction of philosophy to its hand-maiden. Wang We might say that in deconstruction history is always posed as a question, at once urgent, ubiquitous, and insoluble, whereas ideological demystification conceives of its relation to history as an answer, a solution, to its critical hermeneutic. Certainly, this critical truism has special force in Romantic studies, a (...) field very much shaped by the complex relation between deconstruction and ideological critique over the last twenty-five years. But it could just as well be said that the full implications of this relation are especially clarified by the field of Romantic studies, not least because its object of study replays the tensions between these two modes of inquiry. Studying Romanticism means knowing it as a historical period but also knowing it as a figure that stands for something else: an aesthetic practice, a form of consciousness, a political aspiration, an ideology, the possibility of historicity itself. That as a figure Romanticism can be either transhistorical or tied to its historical identity makes its situation all the more complicated and compelling. Romanticism especially dramatizes the interlocking relation between period identity and trope, and the investment of literary studies in that dynamic. Romanticism is the period metaphor that both stabilizes and disrupts the very concept of period metaphors.1 The deconstruction and demystification of Romanticism is very much about the deconstruction and demystification of history, its existence as either question or calculation, trope or immanent being.But even as Romanticism asserts its special relation to history it must also confront an opposite trajectory, how its meaning is best understood through a constellation of other, larger historical identities, such as the Enlightenment and modernity itself. At present the potential disciplinary reorganization of Romanticism into the long eighteenth century is the most vivid academic expression of the question, where does the historical specificity of Romanticism reside, within itself or something larger, or both?Sorting this issue out is certainly a historical proposition, but as Romanticism's special relation to history reminds us, it is also, in Paul de Man's sense, rhetorical. Which is to say, Romanticism's relation to history is, paradoxically, not special-or, more precisely, it is a trope for something pervasive among all the period fields of literary studies, insofar as they remain particular and distinct from one another. This includes those historical entities that might subsume or entangle with Romanticism itself, that would enact historicity by absorbing a field so intent on both the enabling and worrying of historical thought. Approached tropologically, the relation of Romanticism to these larger historical periods is not simply about events and formations that constitute [End Page 15] the boundaries of historical identities. It is also about best identifying and clarifying the workings of figure that create the intelligence of such periodicity.Such labor, what de Man designated innocently enough by the term "reading," calls for its own crossing of a particular set of boundaries. A philosophical text, generated by the entanglement between Romanticism and the eighteenth-century Enlightenment, might contain sections exemplary in their recording of the interstice between trope and historical periodization. Immanuel Kant's short meditation on genius in the third Critique is, like the rest of his book, about the limits and possibilities of the judging subject; his words on genius are also specifically about the artistically creative subject. In resolving the contradictions between aesthetic judgment and creation, Kant transforms the solar light of human genius into the historical genius of the Enlightenment, which is very much the linguistic genius of Romanticism. This all occurs in a register not of consciousness or of historical truth, but of something else before either's constative realization.Kant's text is especially telling in its reflexive expression of this situation, so much so that it helps illuminate the sharp distinction between deconstruction and ideological demystification. The overall purpose of Bricke’s book is to assess the prospects for a coherent theory of mind based on Humean principles. In the light of "recent work in philosophy" Bricke examines what is purported to be Hume’s philosophy of mind as found primarily in book 1 of the Treatise and the first Enquiry. The author also claims that he is "particularly concerned to give an accurate rendering: hence the amount of space devoted to close scrutiny of the texts, and of (...) any arguments Hume presents.... I make little effort to map my interpretation on to those of others.... In the relevant way my account saves the textual appearances".
